International Clinical Fellowship Programme
A supervised clinical post in a London NHS teaching hospital for doctors at or near the end of their national residency programme who hold funding from a sponsoring body.
Overview
Fellows work in their chosen specialty, under consultant supervision, alongside registrars. The post runs for one to two years and is open across all adult and paediatric specialties.
The programme was designed by senior consultants and lead clinicians at the Trust. Fellows are registered with the GMC for the duration of the post. Registration is applied for through the hospital's approved sponsorship scheme, which is an alternative to sitting PLAB.
Entry requirements
A primary medical degree and a completed internship
Both must be complete at the point of application.
A four or five year national residency programme, completed or near completion
Doctors in the final stage of a residency programme are considered alongside those who have finished one.
IELTS or OET at the standard the GMC requires
The certificate must remain valid at the point of review.
At least three of the last five years in clinical practice
Clinical practice is also required for the twelve months immediately preceding the GMC registration application. Research, administrative and non-clinical time is excluded from both figures.
A certificate of good standing
Issued by the medical regulator you are registered with at home.
A sponsoring body prepared to fund the placement
A ministry of health, a government department, a hospital, a clinical institute or a national training body.
Programme structure
Consultant supervision
Fellows are attached to a named consultant and follow a curriculum, taking a full part in the clinical work of that team.
Rotation across clinical sites
Placements rotate between sites, working alongside colleagues from other disciplines and at other stages of their careers.
Workplace-based learning
Learning is primarily through clinical work. Complementary programmes, including leadership, are available in addition.
The clinical week
In most specialties: inpatient and outpatient services, procedures and practical skills, multidisciplinary team meetings, grand rounds and journal clubs.
Case mix
A broad case mix across the specialty. Structured subspecialty work is available in some departments.

Eligibility review

Submission to the hospital
If you are eligible we confirm the placement fee and the hospital's document requirements, then submit your file on your instruction. Submission is not an offer.

Interview and conditional offer
Shortlisted candidates are invited to a video interview assessing communication and clinical competence. Both must be satisfied. A conditional offer letter follows a successful interview. The decision rests with the hospital, and we manage the correspondence between you, your sponsor and the department either way.

Registration and arrival
On appointment we work through GMC registration, the visa route and the hospital's document pack through to your start date.
Scope of the post
This is a clinical post, not an approved training post. It does not lead to a Certificate of Completion of Training, carry a national training number, or provide a route to a royal college qualification.
Fees and funding
The post is not salaried. The placement is funded by your sponsor, and no wage is paid by the hospital.
The fee is quoted in writing following the eligibility review, itemised in full. It is set by the hospital and varies between intakes.
